Can TSW affect children?
Yes, paediatric TSW cases are documented, though less common than in adults. Most corticosteroid treatment in children is supervised, uses lower potency classes and shorter durations, which naturally reduces risk. Problems may arise with prolonged application of stronger corticosteroids on large areas without adequate monitoring [1].
Why children are potentially more vulnerable
Thinner skin — especially in infants, allowing easier penetration. Higher body surface area to weight ratio — approximately 3× higher in infants than adults, leading to proportionally greater systemic absorption. Nappies can act as occlusive dressings, increasing absorption in the nappy area.
TSW symptoms in children
Dramatic worsening after stopping — redness spreading beyond areas of application. Marked restlessness and sleep disturbance. Change in symptom character — burning instead of typical itching. Extensive oozing and peeling beyond normal eczema course.

What the studies say
Prospective study by Fukaya et al.: 75% of infants improved after discontinuation and nearly 25% achieved complete remission. Children: 52% improvement. Adults: 80%. Younger children may have better prognosis [2].
Safe corticosteroid use in children
Weakest effective product — class I (hydrocortisone 0.5–1%). Short courses — max 1–2 weeks. Discontinuation plan agreed with the doctor beforehand.
Alternatives for children
Emollients — the foundation. Calcineurin inhibitors — pimecrolimus from age 2, tacrolimus 0.03% from age 2. Betuldiol® products — gentle, suitable for sensitive skin. Consult your paediatrician.
